Single Member LLC Questionnaire
This single member LLC quesitonnaire will solicit from you the preliminary informaitoin our law firm will need to organize, structure and register the LLC.

Please state the name of your LLC exactly how you intend to use it.
Please provide a detailed summary of the products and or services your LLC intends or actually provides.
If you have yet to establish a business address, or do not intend to, please insert the address of your residence.
Please state your home address details.
A single member LLC may be taxed as a sole proprietorship, S Corp or C Corp. The majority of all new businesses are taxed as sole proprietorship, at least initially.
Member-Managed or Manager-Managed Structure
To retain the maximum asset and creditor protections that the LLC offers, we suggest a manager-managed structure. In short, we will elect you, as the sole member, to manage the internal operations of the LLC.
Election of Assistant Manager
Please state the name of any Assistant Managers. Note, the assistant manager does not need to me a member or owner of the LLC. However, if the assistant manager has manager authority, he or she will be able to act, and make decisions, on behalf of the LLC. manager of the business.
Please provide your business website address. If you do not yet have a business website, simply leave blank.
What amount will you initially contribute to the LLC in exchange for your equity/membership interests? Note, if you will contribute a nominal amount, $1,000 should be the minimum.
Please state the name and address of the individual who will be acting as the registered agent for the business.
Please provide a summary of whether your LLC intends to engage any independent contractors or employees now or in the foreseeable future.
Please state the anticipated gross revenues of the business for its first year of operations.
Please state the anticipated expenses of the business for its first year of operations.
